1. Account Creation & Registration

1.1 Account Requirements

To create an account with Bhadohi Taekwondo Association, you must provide:

  • Full legal name as per identity documents
  • Valid email address
  • Contact phone number
  • Accurate personal information
  • Proof of age (Government-issued ID)

1.2 Account Responsibility

  • You are responsible for maintaining the confidentiality of your account credentials
  • You are solely responsible for all activities conducted under your account
  • You must immediately notify us of any unauthorized access or suspicious activity
  • You agree not to share your account with unauthorized persons
  • Sharing credentials may result in account suspension or termination

2. Declaration of Health & Medical Status

Important Medical Declaration

By signing this declaration, you confirm that you are physically and mentally fit to participate in Taekwondo training. Taekwondo is a physically demanding sport that involves high-impact movements, controlled strikes, and full-contact sparring.

2.1 Pre-Existing Medical Conditions

You must disclose any of the following conditions that may affect your participation:

  • Heart or cardiovascular conditions
  • Asthma or respiratory diseases
  • Joint problems or arthritis
  • Bone fractures or injuries (current or recent)
  • Neurological conditions (seizures, epilepsy, etc.)
  • High blood pressure
  • Diabetes or metabolic disorders
  • Back or spine injuries
  • Neck injuries or concerns
  • Mental health conditions requiring medication
  • Pregnancy
  • Any other chronic illness

2.2 Injury & Medical History Disclosure

You must inform the instructors of:

  • Current injuries or pain
  • Recent surgeries or medical procedures
  • Medications you are currently taking
  • Allergies (including to protective gear materials)
  • Any restrictions on physical activity recommended by your doctor

2.3 Medical Professional Clearance

If you have any serious medical condition, we recommend obtaining written clearance from your physician before beginning training. The Association is not liable for any health complications arising from undisclosed medical conditions.

3. Liability Waiver & Risk Acknowledgment

Waiver of Liability

By participating in training with Bhadohi Taekwondo Association, you acknowledge and assume all risks associated with Taekwondo training, including but not limited to physical injury, permanent disability, and death.

3.1 Assumption of Risk

You understand that Taekwondo training involves:

  • High-impact physical movements and exercises
  • Contact with other participants during practice
  • Full-contact sparring and tournament participation
  • Potential for injuries including bruises, sprains, fractures, and head injuries
  • Risk of long-term health consequences from repeated impact

3.2 Release & Indemnity

You hereby release and discharge Bhadohi Taekwondo Association, its instructors, staff, owners, and facility from any and all claims, liabilities, damages, and expenses arising from:

  • Injuries sustained during training
  • Property damage or loss
  • Accidents or incidents on the premises
  • Medical treatment or hospitalization
  • Negligence of instructors or staff

3.3 Parental/Guardian Consent (For Minors)

Parents/guardians of participants under 18 years of age must:

  • Provide explicit written consent for their child's participation
  • Sign this declaration on behalf of their child
  • Assume full liability for any injuries their child sustains
  • Release the Association from all claims and liabilities
  • Provide emergency contact information and medical details

4. Code of Conduct Acknowledgment

4.1 Participant Conduct

By creating an account, you agree to:

  • Respect all instructors, staff, and fellow participants
  • Follow all safety rules and training guidelines
  • Maintain proper discipline and focus during training
  • Wear appropriate attire and protective gear at all times
  • Attend classes regularly and on time
  • Maintain a clean and injury-free training environment

4.2 Prohibited Behavior

The following behaviors will result in immediate disciplinary action or membership termination:

  • Violence, aggression, or excessive force toward others
  • Harassment or bullying of any kind
  • Discriminatory behavior based on race, religion, gender, or caste
  • Use of abusive, vulgar, or offensive language
  • Disrespect toward instructors or authority figures
  • Bringing weapons or dangerous materials to the facility
  • Use of illegal substances or alcohol
  • Theft or damage to property

5. Emergency & Medical Authorization

5.1 Emergency Medical Treatment

You authorize Bhadohi Taekwondo Association to:

  • Contact emergency services (ambulance, police) if necessary
  • Contact your emergency contact person in case of injury
  • Provide basic first aid assistance
  • Accompany you to a medical facility if injured
  • Share medical information with emergency responders

5.2 Emergency Contact Information

You must provide accurate emergency contact details and update them if they change. Failure to do so may delay emergency assistance.

6. Photography & Media Consent

By participating, you give consent for:

  • Photographs and videos to be taken during training and events
  • These images to be used for promotional, educational, and archival purposes
  • Images to be posted on the website, social media, or printed materials

If you do not wish to be photographed, please notify the instructors in writing.

7. Declaration Form

Declaration & Acknowledgment

I hereby declare that:

  • ✓ I have provided accurate and truthful information in this declaration
  • ✓ I am physically and mentally fit to participate in Taekwondo training
  • ✓ I have disclosed all medical conditions and health concerns
  • ✓ I assume all risks associated with Taekwondo training
  • ✓ I release Bhadohi Taekwondo Association from all liability
  • ✓ I agree to follow all rules, regulations, and safety guidelines
  • ✓ I understand this declaration is binding and legal

Note: For participants under 18 years of age, a parent or legal guardian must sign this declaration.
